Using the checklist and free guide
Questionnaire choices stay in the page’s temporary state. The tool does not submit them, place them in browser storage, or attach them to a contact request. Reloading starts over. The downloadable PDF contains the complete guide without questionnaire answers. Printing or saving your on-screen checklist includes the topics selected from your answers in the copy you keep. No contact details are required to read either.
Email delivery is a separate choice
Email delivery is not connected. The disabled delivery fields do not collect, store or send an address. The proposed delivery permission is for Hollis to send the requested checklist and guide only. It does not request a representative’s call or ongoing marketing. Provider and retention details must be finalized before this option is enabled.
Who handles a request
Hollis operates A Lasting Foundation. If you request a conversation, Hollis receives your information and places it in Kornegay Family Enterprises’ CRM. Administrators and the representative assigned to your inquiry can access it to respond.
What the request contains
Your name, email, optional phone and question, California residency and homeowner answers, chosen contact channels, and the permission you give. The system records the receipt time, form and privacy versions, and campaign attribution when supplied.
How it is used
The information supports your requested conversation about family protection and life insurance. Submission does not request automated calls, texts or ongoing marketing. Please keep account numbers, medical history and other sensitive details out of the form.
Security checks and storage
The planned live form uses Cloudflare Turnstile for bot checks and Supabase for CRM storage. Rate controls use hashed email identifiers. No advertising pixels are configured in this preview. The guide is available without submitting personal information.
Contact and retention
The public privacy contact, retention schedule and request-handling process are still under business review. This draft is not a statement that the live service is ready. See the current contact status.
